Nursing Approach to the Client with Pulmonary Hypertension

Interventions

1. One of the most important nursing interventions is to identify those clients who are at high risk of developing pulmonary hypertension, such as those with COPD, pulmonary emboli, CHF, and mitral valve disease.
2. Obtain history from the client regarding onset and progression of symptoms and existing cardiac or pulmonary disease.
3. Assess presence of symptoms, the most common being dyspnea occurring at first with exertion and eventually at rest. Substernal chest pain is also common as is weakness, fatigue, syncope, and signs of right-sided heart failure.
4. Assess ABG which will show a decreased PaO2 (hypoxemia) .
5. The goals of intervention is to manage the underlying cardiac or pulmonary condition.
6. Hypoxia is the most common cause of the pulmonary vasoconstriction which leads to pulmonary hypertension, so continuous oxygen therapy is the major component of management.
7. Initiate fluid restriction and administer digitalis to improve cardiac function, along with adequate amounts of rest, and the use of diuretics to decrease fluid accumulation.
8. Administer oxygen therapy as ordered and instruct the client and family in safe oxygen use in the home.
